1.The domain:
Have your own domain name (www.mydomain.com), or if you already have a domain name host your blog in a directory (www.mydomain.com/blog).Make sure that you host your blog your self, do not use hosted services because you will not be able to use your own domain name and you are not in control of your content, worst case.
2.The Blog Platform:
Choose a blog platform that you would like to use.There are some things you need to consider when choosing the platform you want to use.Is blogging a hobby or business for me? Will I monetize my blog?
3.Blog Design And Optimization:
How your blog looks is very important, it has to look professional.Easy navigation is also a good thing. Depending on your blogs topic, you need to discover the correct keywords. Optimize your blog with RSS, social bookmarking,tags and so on.
4.Linkbuilding:
You need targeted traffic to your blog.Build links by joining sites like Blogcatalog, Mybloglog, Squidoo and others. Submit your blog and start building your network. Submit your RSS feed and blog URL to blog directories in your niche.Make contact to related bloggers by using the social networking services, forums and discussion threads.There are lots of great marketing related forums out there like Warrior Forum or Digital point to name a few.

5.Monetizing Your Blog:

6.Post Regularly:

I cannot agree more with you on the hosting your own blog part. If you are serious about getting into blogging – never use a free service. Why put in a lot of hard work gaining a good reputation, getting a good following only to find that the free service has gone and changed the rules. Transferring a successful blog to self hosting is very hard work and you wont get to transfer any of the built up rankings.
My advice to all – start by hosting your own blog – they are not that expensive and if you need more disk space, more bandwidth etc, it is not that hard to just back up your site and move it to a new host.

I have seen so many people lose their blogs just by having free hosting.To avoid starting from square one is to have paid hosting and taking a back up once in a while.
